Treatment timeline

Build a clear treatment timeline before your doctor visit.

A treatment timeline is a chronological record of treatments, medications, procedures, appointments, symptoms, and the observations you want to discuss.

It helps preserve the sequence. It does not prove that a treatment caused an improvement, side effect, or other change.

Template

Record the facts that make the sequence easier to review.

1. Treatment or medication

Write the name, start and stop dates, dosage or frequency, and why it was prescribed or attempted.

2. What changed nearby

Record symptom dates, perceived effects, possible side effects, and relevant daily context in your own words.

3. Care events

Add procedures, clinician appointments, test results you were asked to bring, and questions for follow-up.

Prepare the timeline for your doctor visit

Keep the record focused. Mark uncertain dates as approximate. Bring the current medication list and the two or three questions you need help answering. A timeline can show temporal proximity; only a clinician can help interpret whether events are related.
